Notice to Our Patients: Use of AI-Assisted Note-Taking
Ottawa West Medical Centre
To help your physician focus on you during your visit instead of paperwork, our
clinic uses AI tools, Tali AI and Heidi, to help write visit notes.
How it works
With your permission, your visit conversation is safely recorded and written down,
and the AI tool creates a first draft summary. Your physician checks, changes, and
approves the note before it is added to your medical record. The AI does not make
medical decisions. Your physician remains fully responsible for your care and your
chart.

Your privacy
 Your personal health information is not used to teach AI systems.
 Your information is handled following the Ontario Personal Health Information
Protection Act (PHIPA) and federal privacy laws, using protected, secure
systems.

Your choice
Using AI to help with notes is completely optional. If you do not want your visit
recorded, please tell your doctor or our front desk staff. Your doctor will
take notes the usual way. Saying no will not change your care, and you can
change your mind at any time.

The receptionist will ask you to complete a form before your appointment for our records.

Uninsured Services — Now Available Online Through DocNote

Some services aren't covered by OHIP — sick notes, prescription renewals without an appointment, insurance and disability forms, and chart copies. You can now request and pay for these securely online with DocNote, without coming back to the office.

How it works

1. We send you a DocNote request by email.

2. You pay online through the secure link.

3. Your document is delivered to you through a secure link — or, for prescription renewals,

faxed straight to your pharmacy.

Services include

 Sick notes, return-to-work and accommodation letters

 Prescription renewals (no appointment needed) — paid online, faxed to your pharmacy

 Insurance, disability and functional assessment forms

 Chart copies and transfers, sent on a secure link

Your privacy is protected

DocNote is PHIPA- and PIPEDA-compliant; payments are encrypted, and your data stays in

Canada.
